POTS Screening Questionnaire

POTS Screening | Ananda Integrative Medicine

This questionnaire helps your provider assess whether your symptoms may be related to Postural Orthostatic Tachycardia Syndrome (POTS) — a condition affecting how your body regulates heart rate and blood pressure when you stand up. Please answer all questions honestly. There are no right or wrong answers. Your responses will be reviewed by your clinician at your visit.

At-Home Vital Signs

How to measure: Lie down and rest for at least 5 minutes. Take your heart rate and blood pressure while lying down, then stand up and immediately take them again at 1 minute, 3 minutes, and 5 minutes. Use a home blood pressure cuff with a heart rate display if possible. Write down each reading below.

Heart Rate (beats per minute)
Lying down
Standing 1 min
Standing 3 min
Standing 5 min
Blood Pressure (mmHg)
Lying down
Standing 1 min
Standing 3 min
Standing 5 min
Not required: If you don’t have a blood pressure cuff at home, skip the BP fields and just record your pulse. You can use a smartwatch, pulse oximeter, or manually count your heartbeats for 30 seconds and multiply by 2. Important: Stop standing if you feel faint and sit or lie down immediately.

Website Embed Instructions
Option 1 — Direct iFrame Embed (Recommended)

Host this HTML file on your server or a file-hosting service (e.g., Google Drive public link, AWS S3, or your WordPress media library). Then embed it on any page using an iFrame. This is the simplest method and works on WordPress, Squarespace, Wix, and most website builders.

<iframe src=”https://yoursite.com/pots-screening.html” width=”100%” height=”900″ frameborder=”0″ scrolling=”yes” style=”border:none; border-radius:8px;” title=”POTS Screening Questionnaire”> </iframe>

Replace https://yoursite.com/pots-screening.html with the actual URL where you’ve hosted the file. Adjust height as needed — 900px works well on desktop.

Option 2 — WordPress (Block Editor / Elementor)
  1. Upload pots-screening.html via WordPress Media Library, or host it in your /wp-content/uploads/ folder via FTP/SFTP.
  2. Open the page you want to embed it on in the Block Editor (Gutenberg).
  3. Add a Custom HTML block (search “HTML” in the block inserter).
  4. Paste the iFrame code from Option 1 into the Custom HTML block.
  5. If using Elementor: drag an HTML widget onto your section, paste the iFrame code, and click Update.
  6. Preview and adjust height. Publish when ready.
💡 Tip: To send patients a direct link before their appointment, simply link to the hosted HTML file URL in your booking confirmation email or Mailchimp pre-visit automation.
Option 3 — Standalone Page (No iFrame)

If you want this to live as its own URL (e.g., anandaintegrativemedicine.com/pots-screening), upload the file directly to your web host.

WordPress (cPanel / FTP)

Upload via File Manager in cPanel to /public_html/pots-screening.html and access directly at your domain URL.

Squarespace / Wix

Use the Code Block / Embed widget to paste the iFrame — these platforms don’t support raw HTML pages. Use Option 1.

Google Drive (Quick Share)

Upload to Google Drive → right-click → Share → “Anyone with the link.” Send URL directly to patients via email.

Mailchimp Pre-Visit Link

Add the file URL as a button CTA in your appointment confirmation automation: “Complete your POTS screening before your visit →”
